What does the evidence say about melasma treatment?

A 2026 Delphi consensus from the Pigmentary Disorders Society in the Journal of the European Academy of Dermatology and Venereology gathered 38 dermatologists from 11 countries. Photoprotection was called essential, prescription combination creams under supervision were the reference treatment, and peels and microneedling were add ons. Lasers were kept for cases that failed to respond to simpler options, oral tranexamic acid was an alternative, and relapse rates were high.

A 2023 review in Clinical, Cosmetic and Investigational Dermatology (Liu and colleagues) described melasma as multifactorial and reported that sunscreen containing iron oxide reduced recurrence. DermNet adds that microneedling, intense pulsed light and several lasers carry a high risk of relapse.
