A 2025 systematic review in World Journal of Stem Cells included 27 studies of stem cell derived exosomes in hair regeneration. Only three were clinical studies in people, and the rest were laboratory or animal studies. The authors pointed to wide variation in exosome sources, isolation methods and dosing as the main barrier to clinical use.
A second systematic review, published in Clinical, Cosmetic and Investigational Dermatology in September 2025, looked only at clinical evidence. It identified 11 studies up to May 2025, including two randomised controlled trials. All 11 reported improvement in at least one hair measure, with increases in hair density of roughly 9.5 to 35 hairs per square centimetre in studies of mesenchymal stem cell derived exosomes. The authors reported no serious adverse events. The limits were small samples, different designs, varied follow up and no standard protocol, and the authors called for larger controlled trials.
A 2026 scoping review in Medical Sciences compared regenerative options for androgenetic alopecia. It described the clinical evidence for stem cells and exosomes as limited, found PRP the most studied of the group, and noted a lack of high quality studies.
